There is a lot of confusion regarding the hospice discharge and revocation. When is it a discharge, and when is it a revocation? What forms are needed for each? What conditions are required for each? This course reviews the regulations governing both discharges and revocations, the forms required for each, and the role of the hospice interdisciplinary team member in the process. At the end of this course, team members should feel confident in providing education to patients and families and in completing all of the appropriate documentation.
